What it takes

Submit an online application with financial statements, including audited financial statements or tax return for the last fiscal year, or a profit and loss statement and balance sheet if those are unavailable.

Complete DHCS Medi-Cal enrollment and submit recruitment grant-related expenses to receive the first half of the grant payment.

Achieve full credentialing by the Alliance to receive the remaining half of the grant payment.

Complete recruitment and DHCS Medi-Cal enrollment within 12 months of signing the Grant Agreement or the grant will be withdrawn.12 months
