What it takes
Attend the pre-application webinar, which provides an overview of the funding opportunity, timelines, application components, the review process, and a Q&A.
Complete and submit the Application Form, Work Plan, and Budget Worksheet and Justification via the online application link.
Applicants with limited online access may download a Word document application and email the completed form, work plan, and budget to cdp@uihi.org with the subject line: 2026-2027 Sweetgrass RFA: insert agency name.

Funds cannot be used for research, clinical care, or purchasing furniture, equipment, or clinic/patient supplies.
Personnel, travel, supplies, services, and project-related salaries are allowable uses.
